Informed Consent & Waiver

By booking an appointment with us and/or participating in our services, you agree to abide by our studio policy https://www.http://rinyapatthaimassage.co.uk/policy​ and to the following terms:

I hereby for myself, my heirs, executors, representatives, and assigns (hereafter “client”, “I”, or “me”, which terms also include the client’s parents or guardians if the client is under 18 years of age), voluntarily and knowingly consent to participate in massage therapy services (collectively “Services”) provided to me by Rinergy Thai Massage. I certify that I have read, understand, and agree to abide by the full policies of Rinyapat Thai Massage, and I hereby assume all risks involved in my voluntary participation in the Services.

 

I hereby WAIVE, RELEASE, AND FOREVER DISCHARGE Rinergy Thai Massage, their affiliates, managers, members, agents, staff, heirs, representatives, and assigns (collectively “Studio at home”), from any and all claims of negligence, injuries, or losses that I may incur as a result of my participation in the Services. I agree to indemnify and hold harmless the Studio against any and all claims, suits, or actions of any kind for liability, damages, or compensation brought by me or anyone on my behalf, including attorney’s fees and any related costs. I affirm that I am entering into this waiver and release of liability fully of my own volition.
